a) Regenerations inside the TARDIS are freebies (the first Doctor said that the TARDIS would regenerate him, not that he would regenerate);

c) the fourth Doctor's regeneration wasn't a freebie on account of The Watcher's regeneration
d) the sixth, ninth and eleventh Doctor's regenerations weren't TARDIS freebies like the first Doctor's
e) the eighth Doctor's regeneration into War Doctor was a freebie given by the guardians of the flame of Perpetual Boredom
It may be that the doctor has only used 4 'natural' regenerations so far:
3 -> 4, 5 -> 6, 7 -> 8, and [possibly] War Doctor -> 9
It may also be that the Doctor has somehow appropriated the technology that gave the Master an extra cycle of regenerations during the Time War (which I hope we'll see next weekend)Minimum space distance:

My solution would be to say that the limit doesn't apply without a Time Lord council. In Five Doctors the Doctor said the President should ask for another regeneration, strongly implying that the limit was only imposed by them, not natural. No Gallifrey, no limit. That would also explain why the second Doctor said his people could live indefinitely.
